Form Completion Fee
All forms required to be completed by Dr. Rousseau or Dr. Lund will have a $35.00 completion fee required before release. Examples include disability forms and family medical leave paperwork.
No Show Policy
All appointments must be cancelled by 3:00 p.m. the previous day to avoid being charged for a missed appointment. There is a $50.00 "NO SHOW FEE" for missed appointments. After-hour messages may be left at
